Care Coordinator

The Care coordinator is part of our Advance Care Team and works directly with your child’s provider. They provide a variety of services to help patients and their families navigate through the healthcare system and find the available resources that provide optimal outcomes for continuity of care and cost-effectiveness. By working closely with the patient and their family, goals, and needs can be identified and the coordinator will help facilitate and arrange appropriate care with other qualified professionals and resources. The goal of the coordinator is to arrange and coordinate appointments, therapy, and follow-up with specialists based on the patient’s care plan. The coordinator maintains communication with the patient and their family to evaluate whether the plan is effective in meeting the patient's goals and needs with an emphasis on quality of care.
