Current Patient Registration

Please do not create an account without using a PIN that you must receive from us. It will create a duplicate account which will cause errors.

To register a patient for the updated portal, you will need a unique patient PIN provided by the office via your welcome mail or text. If you do not have a PIN, please call the office to request one at extension 1.

Your First Time Logging In

  1. Click "Let's Get Started" button.
  2. Enter your email address and choose a password.
  3. Click "Continue"

Logging In After The First Time

  1. Enter your email address and your password.
  2. Click "Sign In".

At the log-in page it will first ask parental information and then will ask about information for your child. The PIN will be requested to complete your log-in. There is “add another child button” and at that time if you have a PIN for each child you can add them. You can always add your other children at any time when obtaining a PIN is convenient for you. EACH child has a unique PIN. Once you are logged in to your portal account you will be able to see all your children. If you lose your PIN, please call the office.

If your child is over 18, your teen will need to set up their portal account and request to add a parent as a proxy account. You will no longer be able to have a portal account without your child having their own account.
